Nurse Practitioner

The Annual Wellness Nurse Practitioner/Physician assistant will conduct annual wellness visits and create or update a personalized prevention plan to prevent illness based on current health and risk factors and optimize the management of current conditions. This will include screening and performing various tests, screenings, providing general and preventative care and treating illnesses, and collecting family history information. You will also be responsible for counseling patients with education regarding preventative care and prescribed treatments. Compliance with all HIPAA regulations and maintain security of protected health information (PHI) is required.
Job Responsibilities/Duties
Conduct Annual Wellness Visits to completes a comprehensive health assessment to identify all active and chronic conditions, as well as determine all physical, mental and social needs present at the time of the visit.
Perform appropriate screenings, reviews and assessments that include but are not limited to Depression Screening, Medication reviews, Advance Care planning, Fall Assessments, Pain Assessments and Functional Status.
Perform timely and accurate documentation of patients\' medical information and all activity related to patient care in electronic medical record system. Appropriately assign the correct coding for acute and chronic conditions with a special focus on the validation of conditions. Close and complete full and appropriate documentation to reflect the services provided in accordance with company policies and procedures.
Communicates, by phone, in person, in writing, or electronically, with PCPs and Specialists in order to provide comprehensive care or to make referrals, as necessary. Consults with supervising physician as appropriate.
Qualifications
Minimum Education: Graduate of an accredited Masters Degree in Nursing (MSN) program.
Minimum Experience: Two plus years as a Nurse Practitioner.
Req. Certification/Licensure: Current, unrestricted NP/PA CA state license. DEA License and furnishing privileges with the state board. Current state driver\'s license.
Preferred: supporting a very busy Internal Med / Geriatric practice
