DUTIES AND RESPONSIBILITIES

ESSENTIAL:

  • Develop, implement, and evaluate clinical practice guidelines/care paths for patients.
  • Collaborate with all of the health care team, to assist in the provision of quality patient care, and facilitate the identification of potential care problems.
  • Welcome calls from patients and families, answering questions concerning care, and follow-up as necessary.
  • Proactively act as a patient advocate, working to overcome patient barriers, and resolve concerns.
  • Provide educational information and resources to patients, to assist patients and family with coping, and foster self-care activities.
  • Review literature, methodology, outcomes, and implications of nursing/medical research, and disseminate findings.
  • Establish and maintain a knowledge base of on-going clinical research activities.
  • Advocate on behalf of individuals by making referrals to other health care providers, reviews medical reports and provides counseling around making health dietary choices to improve lifestyle.
  • Meet CCNS requirements for computer generated, typed qualitative and quantitative documentation. Including producing progress notes for every client and collateral contact (including both face to face and telephone contacts), writing and completing thorough, accurate notes on behalf of the individual. Computer literacy is essential. Write notes concurrently in the presence of participants.
  • Take blood pressure, measure weight and height, and glucose readings from clients as needed; responds to medical emergencies when they occur on CCNS premises until EMS arrives
  • Works collaboratively with the Psychiatrist and treatment team.
  • Collaborate with Agency and program staff to develop and adhere to on-site emergency response preparedness and readiness protocols.
  • Ensuring that target population has access to service according to contractual requirements and Agency Mission.
  • Monitoring and coordinating psychiatrist psychopharmacology appointments.
  • Follow-up on referrals made on behalf of the participant and respond to all referrals in a timely and professional manner, recognizing the need for effective collaboration in order to insure the highest levels of service provision.
  • Provides weekly group and individual supervision to IHW nursing staff.
OTHER DUTIES:

  • Assist as a liaison with patients and their families to physicians, staff, and other health care professionals, to coordinate patient care across all settings, and ensure high quality outcomes.
  • Participate in in-service trainings and attend relevant workshops/conferences at the request of or with the permission the Clinical Director.
  • Participate, as needed, in Agency response to serious incidents.
  • Collect urine specimens for toxicology’s as needed.
  • Attend weekly team meetings, participates in case reviews and weekly individual supervision.
  • Respond to 24 hour emergency service (BLASK) seven days per week.
  • Participate in all CCNS CQI and QA projects.
  • Performs other duties as assigned.
SPECIFICATIONS FOR EDUCATION/LICENSES/CERTIFICATIONS
  • Bachelor’s Degree Preferred
  • Master’s in Health Care Administration preferred
  • New York State Registered Nurse License and Current Registration
  • Current CPR (BLS), First Aid
SPECIFICATIONS FOR EXPERIENCE AND TRAINING

  • Experience with Mental Health preferred.
  • Clinical Experience preferred.
  • Excellent oral, writing and communication skills.
  • Ability to teach skills to individuals and groups.
  • Free from present litigation that could result  in loss of licensure.
  • Capacity to learn computer skills.
  • Ability to perform skillfully, communicates clearly, discuss assessments with clients.
  • Current knowledge of psychopharmacology and side effects.

SPECIFICATIONS FOR PHYSICAL REQUIREMENTS
  • Ability to travel to multiple sites as needed.
  • Must be able to work flexible hours, including some evenings and Saturdays.
  • Ability to read printed materials and computer screens.
  • Ability to have manual dexterity to operate standard office machines, such as, computers, fax machines, copiers and telephones.
  • Frequent sitting, standing, bending, reaching, and climbing stairs.
  • Ability to lift at least up to 30 pounds.
